ForgeVita LLC operates strictly as a technology and intake routing platform. We do not practice medicine or pharmacy. All clinical evaluations, provider disclosures, professional fee authorizations, and prescriptions are managed independently within the secure partner workflow.
Information We Collect
We collect the following categories of information when you use the ForgeVita platform:
- Identity information: Name, date of birth, and gender.
- Contact information: Email address, phone number, and mailing address.
- Location information: Current state of physical location, used to confirm program routing availability.
- Health history: Information you provide in intake forms for the purpose of routing your request to an independent licensed provider.
- Payment information: Billing details collected and processed through our secure payment partners.
- Technical data: Device information, browser type, IP address, usage data, and cookies collected automatically during platform use.
How We Use Information
ForgeVita uses the information we collect for the following purposes:
- To route your assessment to independent licensed providers configured for your state and requested program;
- To communicate with you about your request, account status, and platform updates;
- To coordinate care with clinical partners and contracted service providers;
- To improve platform performance, user experience, and technical infrastructure;
- To manage your account and respond to support requests;
- To comply with applicable legal and regulatory obligations;
- To maintain platform security, detect fraud, and prevent abuse.
We do not use your health information for targeted advertising. We do not sell your personal health data.
Health Information Workflows
The public ForgeVita website and marketing pages are separate from the secure clinical partner workflow. Health data you provide in the intake form is used to route your request to an appropriate independent licensed provider.
Detailed health information submitted within the clinical workflow — including provider-patient communications, clinical evaluations, and prescription details — is governed by the treating provider's applicable notices, policies, and applicable state and federal law.
ForgeVita does not access detailed clinical records beyond what is required for platform operations, security, and administrative services as described in this Privacy Policy and the Medical Records Policy.
Data Sharing
We share your information only in the following circumstances:
- Independent licensed providers and pharmacies necessary to evaluate and fulfill your care request;
- Contracted service providers who perform functions on our behalf (e.g., payment processing, analytics, infrastructure), under confidentiality obligations;
- Regulators or law enforcement when required by applicable law, court order, or legal process;
- Successor entities in connection with a merger, acquisition, or sale of assets, subject to the same privacy commitments.
We do not sell personal health data for advertising purposes. We do not share your information with third-party advertisers for behavioral targeting on sensitive pages including intake forms, checkout, provider messaging, or medical records.
Cookies & Analytics
We use essential cookies on public pages to provide core platform functionality, maintain session state, and improve performance. We use analytics tools to understand aggregate platform usage and improve the user experience.
We suppress advertising pixels and third-party tracking on sensitive pages, including intake forms, checkout, provider messaging, and medical records pages. Non-essential cookies are not set on these pages.

Data Retention
Clinical records are retained by the treating provider or medical practice for the period required by applicable law, which varies by state and provider type.
Platform account data is retained while your account is active and for a reasonable period thereafter as required for legal compliance, dispute resolution, and business operations.
Analytics and technical data is retained for up to twenty-four (24) months. Payment records are retained as required by applicable financial regulations.
To request deletion of your platform account data, contact us at admin@forgevita.com. Note that we may be required to retain certain data by law even after a deletion request.
Your Privacy Rights
Depending on your state of residence, you may have the following rights regarding your personal information:
- The right to access the personal information we hold about you;
- The right to correct inaccurate personal information;
- The right to request deletion of your personal information, subject to legal retention requirements;
- The right to restrict or object to certain processing of your personal information;
- The right to data portability where technically feasible.
Rights available under the California Consumer Privacy Act (CCPA/CPRA), Virginia Consumer Data Protection Act (VCDPA), Colorado Privacy Act (CPA), Connecticut Data Privacy Act (CTDPA), Utah Consumer Privacy Act (UCPA), Texas Data Privacy and Security Act (TDPSA), Oregon Consumer Privacy Act (OCPA), and Washington My Health My Data Act (MHMDA) are honored for residents of those states. ForgeVita will respond to verified requests within forty-five (45) days, subject to extensions permitted by applicable law.

Your Privacy Choices
- We do not sell your personal information, and we do not share your personal information with third parties for cross-context behavioral advertising or targeted advertising purposes.

- We honor Global Privacy Control (GPC) signals sent by your browser where applicable law requires. If a GPC signal is detected from your browser, we will treat it as a valid opt-out of sale and opt-out of sharing for the applicable session.

ForgeVita is committed to notifying affected individuals within 60 calendar days of discovery of any unauthorized acquisition of their health-related personal information, consistent with the Federal Trade Commission's Health Breach Notification Rule (16 CFR Part 318). A notification, where required, will describe what information was involved, the approximate date range of the incident, what we have done to investigate and mitigate, what steps you can take to protect yourself, and contact information for further questions. Security
We implement commercially reasonable technical and organizational safeguards designed to protect your information from unauthorized access, disclosure, alteration, and destruction. These measures include encryption of data in transit, access controls, and security monitoring.
No security measures are absolute. No method of electronic transmission or storage is 100% secure. We cannot guarantee the absolute security of your information.
